Summary

Reporting to the Director of Social Services or Program Director, the Social Services Supervisor oversees the clinical and case management functions of the Social Service Department, ensuring the ongoing best practice approach of program functioning and compliance with regulatory agency policies and procedures.

Responsibilities

Program/Facilities

  1. Supervisesday to dayoperations and clinical staff in the absence of the Director of Social Services / Program Director.
  1. Supervises and provides leadership and training to social service team members.
  1. Provides administrative and programmatic supervision to case management staff. 
  1. Conducts probationary and annual performance reviews for staff. 
  1. Assesses, trains andcoachescase managers on work assignments and community resources.
  1. Provides support and guidance to case managers in the execution of all housing related tasks.  
  1. Meets individually for weekly supervision with all direct reports.
  1. Supervises andfacilitatesscreening and intake process forclients.
  1. Assigns cases to case managers andscheduleswork according to priority.
  1. Coordinates work and vacation schedule for social service staff
  1. Conducts case conferences, reviewscasesand resolves client issues.
  1. Audits client records and ensures compliance with Urban Pathways, regulatory and funding agencyrequirementsand guidelines. 
  1. Assesses, trains andcoachescase managers on work assignments and community resources.
  1. Documentsincidents and distributes related notices within 24 hours.
  1. Monitors client medications, asrequired.
  1. Reviews service plans, progress notes, and other required client documentation to ensure comprehensiveness and compliance with Urban Pathways andregulatory agency requirements. Signs off all final documents prior to filing in client charts.  
  1. Ensures case managers are adhering to the 360 Chart Auditon a monthly basis.
  1. Ensures housing documentation is completedin a timely mannerfor submission to housing providers.
  1. Manages caseload in the absence of the Case Manager. 
  1. Completes the daily bed respite manifests as mandated byprogram.
  1. Is oncallto address emergency needs on atwenty-four hourbasis.

Administrative

  1. Completes andsubmitsrelevant reports and statisticsin a timely manner. 
  1. Supervises all incident reporting and other client activities to ensure proper documentation and data entry.
  1. Prepares staff schedules and monitor payroll.
  1. Supervises the AWARDS and CARES database to ensure accuracy of client information.
  1. Entersdata,compileandsubmitreportsin a timely manner.
  1. Prepares andsubmitsdaily CARES reporting.
  1. Ensures clients are closed out of the CARES.

Community Relations

  1. Develops andmaintainsrelationships with community services and resources.
  1. Participates in external meetings with fundingagencyand housing providers.

Communication

  1. Communicates verbally and in writing in a professional manner at all times.
  1. Facilitates staff meetings,trainings, and supervisory sessions, asrequired.

Performs all other duties as assigned and as required.

Qualifications

Education and Experience

  • Master’sdegreein a clinical fieldrequired. MSW preferred.
  • Minimum twoyears’of clinical and supervisory experience preferred.
  • Professional license(LCSW)preferred.
  • Experience with homeless andmentally-illpopulation preferred.

Skills

  • Strong written and verbal communication skills.
  • Strong interpersonal skills and ability to work independently or in a team setting. 
  • Strong teambuilding and coaching skills and experience. 
  • Strong problem-solving skills and resourcefulness.
  • Strong computer skills and advanced knowledge of Microsoft office (e.g.Word, Outlook, Excel, etc.).

Other

  • Required toworka flexible schedule including weekends,holidaysand evenings.
  • Valid driver’s licensewithclean driving recorda plus.

Physical Capabilities

  • All programs require field work, home visits, client escorts, and use of public transportation.
