We are currently seeking qualified candidates for the following position:

Position: Social Work Supervisor / Client Care Coordinator

 

The Social Work Supervisor / Client Care Coordinator will enhance existing services to include a thorough assessment of families’ long-term goals at intake and development of service plan to incorporate small achievable tasks leading to permanent housing with enrollment and/or referrals to community resources.

Location: Brooklyn, NY 11236

 

Minimum Qualifications:

  • Master’s Degree in Social Worker with current LMSW licensure,
  • At least 2 years of experience including in a supervisory capacity,
  • Knowledge of adult, child and adolescent development, emotional/behavioral health and mental health, family dynamics, parent-child relationships, diagnostic classifications.
  • In addition, the position requires a reasonable combination of skills in the following areas: strengths based, solution focused, and family centered practice; experience working with diverse client populations; knowledge of child welfare policies, regulations, safety, permanency, and well-being.

 

Social Worker Supervisor / Client Care Coordinator Principal Responsibilities:

  1. Supervise Social Workers/Client Care Coordinators
  2. Manage a small caseload
  3. Assign case to client coordinators for newly admitted families to be assessed immediately upon admission
  4. Develop schedule for Client Care Coordinators to be onsite 7 days a week including evening hours
  5. Manage program record keeping, data collection, and evaluation on the effectiveness of client care coordinators services for families in shelter.
  6. Ensure that quality services and interventions are being provided and develop a system for monitoring.
  7. Provide on-call crisis intervention support.
  8. Develop community linkages
  9. Develop and implement onsite support groups for families
  10. Perform other related duties as assigned.
