Requirements
Role Overview
Perform in-depth psychiatric evaluations and ongoing assessments for residents in skilled nursing facilities.
Formulate tailored treatment plans that align with each patient’s diagnosis and clinical needs.
Manage psychiatric medications, including initiation, monitoring, and adjustments as appropriate.
Partner with physicians, psychologists, social workers, and nursing staff to coordinate comprehensive patient care.
Provide therapeutic support, counseling, and guidance to both patients and their families.
Educate patients and caregivers on mental health conditions, treatment options, medication use, and coping strategies.
Document all clinical encounters accurately and ensure compliance with state and organizational standards.
Take part in team discussions, case reviews, and initiatives focused on improving clinical outcomes and care delivery.
Qualifications
Active New York Nurse Practitioner license with certification as a PMHNP.
Ability to manage an independent caseload with professionalism and sound clinical judgment.
Strong assessment and diagnostic abilities; solid understanding of psychopharmacology and psychiatric interventions.
Outstanding communication skills, with the ability to build trust and collaborate effectively with patients, families, and interdisciplinary teams.
Comfortable using electronic health records (EHRs) and general computer applications.
