PRINCIPAL DUTIES AND RESPONSIBILITIES

1.      Interview patient and/or family to obtain psychosocial information and assess the patient’s functional level, relative to safety and/or discharge planning needs.

2.      Promote a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c)

3.      Make Customer Service a priority, treating customers (patients, visitors, and co workers) in a professional manner exercising courtesy and tact. Effectively communicates with patients and others about SBH services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in SBH in a timely fashion.

4.      Adhere to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct.

5.      Formulate an assessment and treatment/discharge plan that will             highlight a clinically realistic, and sensitive understanding of the         patient’s needs.

6.      Provide direct social work/discharge planning services including community resource advocacy as well as keep ongoing professional        rapport with community agencies as needed. Conducts meaningful communication   with physicians and conducts timely follow up for completion of required documentations to facilitate discharges.  

7.      Documents all work activity in the patient’s medical chart in and Allscript in accordance with departmental and regulatory guidelines.

8.      Participates in scheduled multidisciplinary rounds/meetings.

9.      Engage with patient and/or family members with respect to cultural differences and sensitivity to individuality.

10.  Demonstrate consistent attention towards patient confidentiality.

11.  Counsel patients, families, and significant others in the areas of expectation regarding life style changes and functional coping patterns.

12.  Complete and submits monthly statistics timely.

13.  Attend all scheduled Care Transition Social Work Departmental meetings, supervisions and obtain consultation when needed.                            

14.  Speak to patients, families and significant others with age appropriate   conversation and demeanor.

15.  Attend  and participate in White Board Rounds,  discharge planning meetings and patient conference, etc., as needed

16.  Assist in fulfilling the day-to-day responsibilities of the department, i.e. staffing assignments.    

17.  Schedule and provides oversight of new staff orientation, assists in orientation, makes recommendations for appropriate release of new staff and/or the need for additional training with extension of orientation period.

18.  All other duties as assigned.

Qualifications
  1. Education Required: Master’s Degree in Social Work from an accredited school of social work.  Participation in continuing education activities is expected.

New York State Certification (CSW)

 

Experience/Skills Required:   (2) years’ experience in a hospital setting, and related to the department the worker will be assigned
