Peer Educator - Access to Healthcare

Community Healthcare Network (CHN) is a not-for-profit organization providing more than 65,000 New Yorkers with primary and behavioral healthcare, dental, nutrition, wellness, and needed support services.

POSITION SUMMARY:

Responsible for conducting outreach, providing general risk reduction, providing referrals to needed services, distributing HIV/AIDS information and condoms, participating in training and support activities, utilizing data forms to document and report activities performed.

DUTIES AND RESPONSIBILITIES:

1. Conducts community outreach and door-to-door outreach in targeted areas identified by the Program Manager to meet grant targets.
2. Participates in community health and community fairs as an agency representative.
3. Provides referrals of clients to necessary health and support services in the community, including making appointments for clients at CHN clinic sites.
4. Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.
5. Performs other related duties, as assigned.

OTHER FUNCTIONS:

1. Identifies community resources and makes contact with such agencies.
2. Escorts clients to necessary services, as appropriate.
3. Documents clients reached through community outreach and report this in AIRS.
4. Participates in training, in-services, and support activities for peer educators.
5. Distributes HIV/AIDS educational literature to clients and agencies.
6. Performs other related duties as assigned.

Certificate/License required:

• High School diploma or G.E.D. equivalency

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

• Should be a member of the targeted communities and of the targeted service groups (a ‘peer’).
• Good communication skills and experience with HIV issues.
• Good verbal and written communication skills.
• Knowledge of geographic area of work site and of language appropriate to community.
• Demonstrate ability to work with HIV-infected individuals.
• Interest in and conviction of purpose and aims for all programs in the clinic.
• Respect for the rights of individuals to make their own decisions about their medical care.
• Ability to communicate easily and display a cordial manner towards individuals from a variety of socio-economic, cultural and religious background.

Tuesday : 9 AM- 5 PM

Wednesday: 9 AM- 5 PM

Thursday: 9 AM- 4 PM
