Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, in instill hope and to strengthen resiliency. The Social worker is responsible to carry a caseload and provide case management services to individuals. Provide social work interventions based on assessment of residents need and case service plan. * Conduct assessments, unit inspections, and complete clinical documentation as required in a timely manner. Document all resident contact in progress notes. * Conduct services with residents using evidence base practices specifically Critical Time Intervention. * Complete all intake assessment and coordinate intake process for new residents * Complete all initial required assessment upon admission and annually based on funders requirements. * Prepare integrative Person Centered Service plan, based on assessments from referral source and in collaboration with resident. * Maintain an up to date clinical charts at all times. * Initiate social work modalities through appropriate use of agency and community resources, including advocacy and crisis intervention. * Facilitate groups on and off site. * Provide linkage services to residents. * Provide crisis management and interventions. * Coordinate all services for resident, including day programs, medical care, transportation, and community inclusion (Person Centered Planning). * Conduct meetings to ensure proper treatment, progress in program and coordination of goals. * Attend case conference at day program or hospitals. * Work with Direct Care Workers to enhance communication and cooperation between them and resident families. * Participate in audit preparation. KEY ESSENTIAL FUNCTIONS: * Obtain, analyze and evaluate case data. * Carry out treatment processes. * Analyze and evaluate course and results of treatment processes. * Record material and case summaries. * Utilize Agency resources as well as community resources to perform related tasks.
