Job DetailsJob Location: Academy (West Harlem) - New York, NY 10031Position Type: Full-Time ExemptSalary Range: $70,000.00 - $75,000.00 Salary/yearTitle:                           Social Worker-JISH Unit:                            Housing Reports to:                  Senior Director of Transitional Housing Salary Range:             $70,000 - $75,000 Status:                         Full Time; Regular; Exempt Location:                    Manhattan Location Days/Hours:               Monday - Friday, Days/Evenings   Position Summary: The Social Worker will provide ongoing clinical support to Justice Involved housing participants with a history of homelessness, justice impacted and may have mental health and/or substance use challenges.  The Social Worker will work alongside the case managers and housing specialists to engage participants in need of permanent supportive housing.  This includes supporting the program modalities and individual services.    Essential Duties and Responsibilities: Provide support to the housing specialist, case managers to ensure housing applications are completed timely.  This includes the timely completion of psychosocial assessments for CAPS applications. Review incoming referrals and implement treatment plan recommendations for the case management team. Support the program for group facilitation and engagement.  Training on trauma and other factors that impact engagement with participants.  Assist the Senior Director with special projects related to the Justice Involved Supportive Housing (JISH) contract.     Provide clinical support to case managers as they work with the senior residents. Provide clinical and counseling support to participants as needed. Attend clinical ARC meetings to assess appropriateness for permanent housing placement.  Request additional information from referral providers and court advocates to ensure program staff can support diverse needs. Work with the Intake Department on cases that require additional support such as ICM, CHS and ACT.   Work cohesively with Behavioral Health, SRP, and DOCS to ensure that wraparound supports are in place. Assist with family reunification support Work with external providers to ensure quality implementation of resources for housing placements.  Participant support: Conduct comprehensive intake assessments covering substance use, mental health, housing history, legal involvement, and social supports. Connect clients to mental health, substance use treatment, medical services, and legal aid as needed.  Utilize strength based, client centered, harm reduction and trauma informed practices (ex. Motivational interviewing)  Participate in interdisciplinary team meetings to ensure coordinated care. (ex. facilitate case conferences) Housing: Help participants understand and uphold lease agreements, including conflict resolution with neighbors or landlords.  Monitor and address housing issues that may lead to eviction risk (behavioral issues, substance use etc.) Facilitate life skills groups (e.g., budgeting, housekeeping, communication) to support independent living             Other duties as assigned.   QualificationsQualifications: MSW required, LMSW preferred. Minimum of one year experience working with the senior homeless population preferred Some work or internship experience in dealing with either significant psychiatric issues, trauma or incarceration; Bilingual Spanish-speaking preferred; Strong computer skills, particularly in electronic health records; Excellent resident-engagement and counseling skills;  Excellent clinical note writing and time management skills; We seek talented, dedicated individuals from all walks of life who possess a strong commitment to this mission.  Relevant personal experience is a plus.   Travel Requirements:  May require travel     Physical Demands: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
