Description

Title: Senior Care Manager- Position located in Newburgh


Reports to: Care Coordination Program Manager


Job Objectives:

Provide care management to individuals enrolled in the Health Home Care Coordination Program, while also mentoring other care managers within the department.

Perform duties while actively promoting an environment consistent with MHA鈥檚 Cultural Equity Policy and WELCOME Orange.

Responsibilities:

路 Attend bi-weekly Care Coordination team meetings.

路 Attend trainings and meetings relevant to Health Homes.

路 Deliver care coordination services as outlined under Health Home Model for Care Coordination for a caseload of between 35-45 program participants.聽

路 Complete New York State Community Mental Health (CMH) assessments and reassessments.

路 Complete monthly scorecard data collection information.

路 Maintain all client records electronically through Foothold Technology and FCM Electronic Record Keeping.

路 Maintain all client files according to program policies and procedures.

路 Establish and maintain linkage with appropriate community agencies and service providers.

路 Represent MHA at meetings, as necessary.

路 Assist with internal audits of participant charts.

路 Provide oversight to other care managers in the field, as needed.

路 Assist staff when program participants may be experiencing a crisis and/or responding to phone calls from participants in crisis.

路 Answering general care management and health home questions for staff and ensuring a manager or supervisor is informed if the question or situation is more complex.

路 Assisting with training and/or shadowing new care management staff.

路 Reviewing time management strategies and deadlines with staff, as needed.聽


Requirements

Qualifications:

路 Bachelor鈥檚 Degree in any of the following: child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation/recreation therapy, rehabilitation, social work, sociology, or speech and hearing.聽

OR

路 NYS licensure and current registration as a Registered Nurse and a bachelor鈥檚 degree.

OR

路 A bachelor鈥檚 level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses.

OR

路 A Credentialed Alcoholism and Substance Abuse Counselor (CASAC).

AND

路 Must have a valid driver鈥檚 license and access to a reliable, personally owned vehicle.

Experience:

路 Two years of experience (a master鈥檚 degree in a related field may substitute for one year鈥檚 experience) either:

路 Providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse.聽

OR

路 Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse to abroad range of services essential to successfully living in a community setting.

路 Bilingual Spanish / English preferred.

路 Effective written and oral communication skills.

路 Experience with Health Homes highly preferred.

路 Knowledge of Medicaid procedures highly preferred.

路 Experience with Electronic Health Record Program(s) required.

路 Experience working with Excel and Microsoft Word preferred.聽

路 Ability to work independently with minimal direct supervision.

路 Must be organized, self-motivated and have the ability to coordinate multiple tasks simultaneously.

路 Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.

路 Valid NYS Driver鈥檚 License and ability to drive with current driving record acceptable for agency insurance coverage.

Other Duties:

路 In the spirit of teamwork, answer MHA鈥檚 main office phone as needed.

路 Participate in MHA鈥檚 committees as needed.聽
