Licensed Master Social Worker (LMSW)

Always Compassionate Health is a Licensed Home Care Services Agency providing exceptional community-based nursing care across the Newburgh area. We are seeking a Licensed Master of Social Work for evaluations, nursing guidance, and monthly chart reviews of ventilation patients we are currently servicing.

Qualifications/Education:
• NY State LMSW license
• 2 years working as a social worker in a clinical environment.
• Experience working in Home Care as a Social Worker preferred.

Responsibilities:
• Assess client home environment, factors impacting provision or barriers to client’s care.
• Make recommendations for community and other resources for clients and family which facilitate the established plan of care.
• Participate in client conferences as required.
• Must be able to lift 50lbs.
• Must be able to stand/work for extended periods.
• Additional responsibilities as required.

Salary:
• $50/hour
