This position reports to the Clinical Director (Chief of Nursing).
The Home Health Manager plays a critical leadership role in overseeing the delivery of high-quality home health care services to patients within their own residences. This position is responsible for ensuring compliance with healthcare regulations, managing clinical and administrative staff, and coordinating care plans that meet the individual needs of patients. The manager will work closely with interdisciplinary teams to optimize patient outcomes while maintaining operational efficiency and financial sustainability. They will also serve as a liaison between patients, families, healthcare providers, and regulatory bodies to ensure seamless communication and service delivery. Ultimately, the Home Health Manager ensures that the organization provides compassionate, effective, and patient-centered care in a home setting.
Minimum Qualifications:
- Minimum of 3 years of experience in home health care or a related healthcare management role.
- Strong knowledge of home health care regulations, including Medicare and Medicaid requirements.
- Proven leadership and team management skills with experience supervising clinical staff.
Preferred Qualifications:
- Bachelor's degree in Healthcare Administration, Nursing, or a related discipline.
- Current and valid Registered Nurse (RN) license if applicable, or relevant professional licensure.
- Certification in Home Health Care Management or Case Management (e.g., CHCM, CCM).
- Experience with electronic health records (EHR) systems and healthcare data analytics.
- Familiarity with quality improvement methodologies such as Lean or Six Sigma.
- Experience working with diverse patient populations and community health initiatives.
Responsibilities:
- Supervise and support home health care staff including nurses, therapists, and aides to ensure quality patient care.
- Develop, implement, and monitor individualized care plans in collaboration with healthcare professionals and patients.
- Ensure compliance with federal, state, and local regulations as well as accreditation standards related to home health services.
- Manage scheduling, staffing, and resource allocation to optimize operational efficiency and patient satisfaction.
- Conduct regular performance evaluations, provide training opportunities, and foster a positive work environment.
- Oversee patient admissions, discharges, and transitions of care to maintain continuity and quality of service.
- Monitor clinical outcomes and quality metrics, implementing improvements as necessary.
- Serve as the primary point of contact for patient and family concerns, resolving issues promptly and effectively.
- Prepare and manage budgets, financial reports, and billing processes to ensure fiscal responsibility.
- Collaborate with external healthcare providers, insurance companies, and community resources to support patient care.
