Job DetailsJob Location: Newton, KS 67114 Join our team at Prairie View, Inc. to support individuals in addiction recovery by coordinating their care, fostering their growth, and connecting them with vital community resources. 1. Direct Client Care & Case Management This is the core function of the role, focused on providing supportive services to adult and adolescent clients in addiction recovery. Provide continuous, supportive care coordination and crisis intervention. Develop, facilitate, and monitor initial and periodic treatment plans (every 90 days), coordinating with the therapist for clinical oversight. Assess client strengths and needs to facilitate community integration and autonomy. Educate clients on service choices and help them build skills for relapse prevention and managing emotional disturbances. Act as a liaison by building relationships with community agencies to create a supportive network. Ensure client and worker safety by maintaining appropriate boundaries and implementing knowledge of risk factors and interventions. 2. Administrative & Compliance Duties This covers all necessary record-keeping, authorization, and internal collaboration. Maintain adequate and timely clinical documentation, ensuring progress notes and records meet internal and external standards. Coordinate with Utilization Review staff to ensure service authorization is in place and accurately documented on the Treatment Plan. Participate in required supervision, team meetings, and departmental meetings. Manage a schedule and establish priorities according to patient and agency needs. 3. Professional Development & Quality This section covers the continuous learning, certification, and quality improvement activities required for the position. Complete all necessary continuing education, workshops, and in-services (e.g., Infection Control). Complete approved State of Kansas training, including Person-Centered Case Manager (PCCM) training. Participate actively in the organization's Quality Assurance & Performance Improvement (QAPI) activities. 4. General Requirements & Organizational Values This covers the logistics of the role and the behavioral expectations of the employee. Patient Transportation: Transport patients as required for service delivery. QualificationsPOSITION REQUIREMENTS: Education: Bachelor’s degree. Experience: Minimum of two years working with adults who have substance use disorder and/or severe and persistent mental illness. Certification: Must hold or be willing to obtain and maintain the State of Kansas Person-Centered Case Manager Certification (PCCM), including required Continuing Education Units (CEUs). Skills: Excellent verbal, written, and listening communication skills. Ability to work effectively and provide support to adults with substance use disorder/mental illness. Proficient at basic computer and keyboarding skills. General Requirements: Must be at least 21 years of age. Must be able and willing to work a flexible schedule to meet patient and organizational needs. Must pass required background checks (KBI, Central Registry, and DCF/SRS).
