Job DetailsJob Location: Main Office - Newton, MA 02459Per Diem Weekend Nurse Liaison  Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks a Per Diem Weekend Nurse Liaison to join its expanding weekend nurse team. This is an on-site position located at Newton Wellesley Hospital.   Job Responsibilities: The position of Per Diem Weekend Nurse Liaison is to provide information about hospice services to patients, families, and hospital-based staff, to conduct clinical assessments, coordinate hospital discharge plans, and maintain professional relationships within hospitals, with a focus on the assigned hospital(s).    Schedule and Weekly Hours: Saturday and Sunday, flexible shifts.   Education, Skills and Experience: Licensed by the Commonwealth of MA as a Registered Nurse. At least two years experience preferred in community health, oncology, hospice, or palliative care. Excellent communication and interpersonal skills Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families Highly skilled at providing patient assessments and evaluations.
