Chandler Hall is known for providing high-quality care and services to older adults in Newtown, Bucks County. Our continuum includes Community Living, Personal Care, Independent Living, Skilled Nursing, Respite Care, and Home Hospice.

 Position Overview

Chandler Hall is seeking an experienced and compassionate Registered Nurse Case Manager to join our Home Hospice team.

The RN Case Manager coordinates and provides hospice nursing care for an assigned caseload of patients. This position works closely with patients, families, physicians, facilities, and the hospice interdisciplinary team to manage symptoms, support goals of care, and help patients remain as comfortable as possible throughout the end-of-life journey.

The RNCM must be able to provide care throughout Bucks County, Montgomery County, and Northeast Philadelphia.

Responsibilities

  • Manage and coordinate care for an assigned hospice caseload.
  • Complete comprehensive and ongoing nursing assessments.
  • Develop, implement, evaluate, and update individualized plans of care with the patient, family, physician, and interdisciplinary team.
  • Provide scheduled and as-needed nursing visits based on patient needs.
  • Assess and manage pain, symptoms, medications, functional changes, and safety concerns.
  • Communicate significant changes in condition and obtain physician orders when indicated.
  • Educate patients and caregivers about medications, symptom management, disease progression, safety, and when to contact hospice.
  • Coordinate care with physicians, facilities, pharmacies, medical equipment providers, contracted vendors, and other members of the care team.
  • Participate in interdisciplinary team meetings and contribute to care-planning decisions.
  • Support hospice eligibility reviews and complete admission, certification, recertification, transfer, and discharge documentation as applicable.
  • Maintain accurate, complete, and timely clinical documentation.
  • Assist patients and families with advance care planning and decisions related to their goals of care.
  • Recognize psychosocial, emotional, cultural, and spiritual needs and coordinate support from the appropriate interdisciplinary team member.
  • Provide compassionate support to patients and families experiencing grief, loss, and changing care needs.
  • Supervise hospice aide and other delegated services when assigned.
  • Follow professional nursing standards, Chandler Hall policies, and applicable hospice regulations.

Qualifications

  • Current Registered Nurse license in the Commonwealth of Pennsylvania.
  • At least one year of hospice nursing experience.
  • Certified Hospice and Palliative Nurse certification preferred.
  • Strong knowledge of hospice philosophy, end-of-life care, and pain and symptom management.
  • Ability to complete patient-centered assessments and develop effective plans of care.
  • Strong clinical judgment and the ability to recognize and respond to changes in patient condition.
  • Compassionate and respectful communication with patients and families facing serious illness, grief, and loss.
  • Ability to work independently in community settings while collaborating closely with the interdisciplinary team.
  • Strong organization, care coordination, and clinical documentation skills.
  • Ability to maintain appropriate professional boundaries.
  • Reliable transportation and the ability to travel throughout the assigned service area.
