Provide patient/caretaker with adequate knowledge to make informed decisions and judgments by teaching preventive care and helping family identify health problems. Promote patient/family independence in accepting responsibility for care. Provide supportive physical, emotional and spiritual care. Evaluate home/family safety. Make recommendations for improvements and elimination of hazards. Utilize the nursing process in implementing plan of care for patients and their caregivers. Contribute to the assessment of community health needs, health education, and participate in preventive health activities. What You’ll Bring: Graduate of an accredited NLN School for Practical Nursing.  Satisfactory completion of approved course in pharmacology. 2 to 3 years of recent acute care nursing or equivalent experience. Community health nursing experience desirable. Current LPN license in the State of Maine & CPR Certification. Must have reliable transportation with insurance and a current Maine Driver’s license.
