RN Home Care Supervisor

Liberty Home Care is looking for a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor! 
 
Schedule: Monday - Friday 8:30am-4:30pm (37.5 hours per week)
 

Duties & Responsibilities:

• Provide assessment of newly referred patients and assess health needs in home care setting
• Work with patient to create a patient centered plan of care
• Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
• Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services
• Provide supervision and training of PCA/HHA staff
• Assist with the on-call rotation (stipend provided)

Qualifications:

• NYS Licensed Registered Professional Nurse
• Minimum of Associate's Degree required
• One year Nursing experience required
• Home care experience a plus
