Job DetailsLevel: ExperiencedJob Location: Noble Hospice (OK) - NOBLE, OK 73068Salary Range: $75,000.00 - $82,000.00 Salary/yearTravel Percentage: Up to 50% Job Shift: DayJob Category: Health Care1.  Primary responsibility for assigned Hospice patients for Case Management to provide all nursing care needed.  Includes assessment, planning, implementing       and evaluating patients, family, home environment, disease progress, etc. 2.  Providing supportive care and overseeing appropriate services in accordance with Physician's orders and the Interdisciplinary team plan of care. 3.  Regular meetings with hospice nursing staff care team to support and review problems.  Also maintain regular communication with hospice team. 4.  Supervisory duties of all other disciplines in assigned hospice patient homes. 5.  Initiates communication with physicians, other hospice staff, family members as needed to coordinate optimal care and use of resources for hospice patients         and their family members. 6.  On-call rotation to provide hospice staff care for 24/7 coverage.  Additional pay for all on-call time hours and visits. 7.  Provide appropriate nursing care and support at hospice patient's time of death.   QualificationsCurrent Registered Nurse license approved by the Oklahoma Nursing Board of Regents.   At least one year experience in either home health or acute-care setting with experience in elderly, middle aged, and/or pediatric care.
