JOB PURPOSE:
Provides pharmaceutical services to patients receiving care in post-acute, long term, or retail settings in a manner that delivers superior patient care and excellent customer service.聽 Monitors and manages the prescription process and daily activities of the closed-door long-term care pharmacy in accordance with company, state and federal policies, laws and regulations.
ESSENTIAL JOB FUNCTIONS, DUTIES, AND RESPONSIBILITIES:
Schedule: Day shifts rotate between 7a-3:30p, 11a-7:30p & 12:30-9p. Weekends will be either once a month or every other (Sat & Sun 7a-3p)
路聽聽聽 Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients needed.
路聽聽聽 Evaluate prescriber medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Make recommendations for medication therapy changes to prescribers as appropriate.
路聽聽聽 Collaborate with the healthcare team to provide clinical and technical support as needed.
路聽聽聽 Document all communication with physicians, nurses, and other members of the healthcare team.
路聽聽聽 Take responsibility for accurate intake and efficient fulfillment of prescription orders.
路聽聽聽 Obtain, as necessary, and maintain all required documentation in accordance with federal and state regulatory agencies and Pharmacy Services policy.
路聽聽聽 Provide and document interim Medication Regimen Reviews for all new admit or readmit patients.
路聽聽聽 Implement and document all approved therapeutic substitution medication orders.
路聽聽聽 Provide appropriate clinical assessment prior to contacting healthcare center.
路聽聽聽 Demonstrate effective and professional leadership skills.
路聽聽聽 Document and investigate errors and variances.
路聽聽聽 Take ownership and accountability for the successful completion of all pharmacy production duties and achievement of pharmacy location goals.
LICENSURE, CERTIFICATION, EXPERIENCE AND EDUCATION REQUIREMENTS:
路聽聽聽 Graduate of ACPE accredited School of Pharmacy.
路聽聽聽 Current licensure in the state of practice.
路聽聽聽 Long-term care pharmacy experience preferred.
路聽聽聽 Experience in patient care and medication management, drug therapy monitoring, drug and medical supply procurement, inventory control management, and performance improvement.
路聽聽聽 Ability to clearly communicate medication information to professional practitioners and the general public.
