As a Home Health Physical Therapist, you'll develop individualized treatment plans, provide hands-on care, and build meaningful relationships with patients and families while enjoying the flexibility and autonomy that comes with community-based care.

What You'll Do

• Evaluate patients and develop personalized treatment plans.
• Improve mobility, strength, balance, and overall function.
• Educate patients and caregivers on exercises, safety, and disease management.
• Monitor progress and adjust treatment plans to maximize outcomes.
• Collaborate with physicians, nurses, and interdisciplinary team members.
• Complete timely and accurate clinical documentation.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license.
• CPR certification.
• Valid driver's license and auto insurance.
• Home Health experience preferred.
• HomeCare HomeBase experience preferred.
