Job DetailsJob Location: 73069, OK 73069Position Type: Full TimeHaven Home Health is seeking a compassionate, licensed Physical Therapist (PT) to join our growing team. In this role, you will provide medically prescribed physical therapy services to patients in their homes or care facilities. You’ll help individuals recover from injury, improve mobility, relieve pain, and prevent disability—all while supporting their independence and quality of life. Qualifications Current Physical Therapist (PT) license in the state of Oklahoma Bachelor’s degree in Physical Therapy required Minimum 6 months supervised clinical experience required; 1–2 years in home health or skilled nursing preferred Recent experience in direct patient care strongly preferred Valid CPR certification Valid driver’s license, reliable transportation, and active auto liability insurance Ability to travel throughout the service area in all weather conditions Excellent verbal and written communication skills Comfortable using EMR/Point of Care systems and basic computer applications Key Responsibilities Review physician referrals and evaluate patient medical records to determine the need for physical therapy Conduct assessments including strength, mobility, sensory perception, functional capacity, and endurance Develop individualized treatment plans based on evaluation findings Administer therapeutic exercises, manual therapy, and physical agents Teach and assist patients in proper use of assistive devices (crutches, canes, walkers, prosthetics, etc.) Evaluate patient progress and adjust treatment plans for optimal outcomes Document care, progress, and patient responses accurately in the EMR Educate patients and families on home programs to support ongoing recovery Collaborate with interdisciplinary healthcare team members to coordinate care Supervise Physical Therapy Assistants as assigned Participate in staff meetings, in-services, and ongoing training Travel daily to patient homes; occasional overnight travel may be required Ensure compliance with Progressive Home Health policies, emergency preparedness requirements, and all state/federal regulations Assist in training/orientation of new staff as needed Evaluate, fit, and adjust orthotic/prosthetic devices and communicate needs to the Orthotist/Physician Supervisory Responsibilities This position may supervise Physical Therapy Assistants, including directing daily activities, providing instruction, evaluating performance, and ensuring adherence to policy and best practice standards. Functional & Physical Requirements Ability to maintain focus and professionalism during frequent interruptions Flexibility to adapt to changing tasks and patient needs Ability to lift up to 50 lbs., assist with patient mobility, and perform hands-on therapy Adequate hearing, vision (including color vision), and physical stamina to perform job duties Comfortable entering various home environments and community settings Why Join Us?
