What You'll Do

As a Case Manager, you will:

• Work as part of a multidisciplinary team to provide holistic, person-centered services.
• Assist individuals in developing and achieving recovery goals.
• Coordinate services and connect participants with community resources and supports.
• Build strong relationships with community agencies and service providers.
• Advocate for participants to ensure their needs are met and barriers are addressed.
• Provide community-based case management services throughout the service area.
• Transport participants to appointments and community resources when needed.
• Complete assessments, service plans, reviews, and required documentation.
• Maintain accurate and timely records in accordance with agency and funder requirements.
• Collaborate with clinical staff, medical providers, and community partners to support positive outcomes.

Qualifications

Required

• Two (2) years of experience providing support to individuals in behavioral health, addiction recovery, unhoused services, or IDD/BHH programs
• Intermediate computer skills
• Strong communication and organizational abilities
• Ability to work independently and in community-based settings

Preferred

• Bachelor's Degree in Human Services or a related field
• One (1) year of case management experience
• Knowledge of behavioral health resources and community support systems

Schedule

• Full-Time
• Primarily community-based work throughout Montgomery County
• Flexible schedule based on participant and program needs
• Some evening or weekend availability may be required

Physical Requirements

• Ability to lift up to 50 pounds
• Ability to stand, sit, walk, bend, reach, and climb stairs as needed
• Ability to travel throughout the community and transport participants as required
• Ability to perform essential job functions with or without reasonable accommodation
