Brief summary of purpose:

Responsible for the provision of speech therapy services to SE participants. Performs assessments of participants’ physical and cognitive abilities and provides treatment as indicated. Participates in care planning and interdisciplinary team meetings as requested by Lead therapist.  Provides all services in accordance with applicable regulations as well as FCHP and SE policies and procedures.  

Responsibilities

Primary Job Responsibilities: 

  • Assures that all speech therapy assessments are completed and documented in a timely manner.
  • Works in collaboration with the interdisciplinary team (IDT), participants and caregivers to meet participants’ speech therapy needs.
  • Assures that problems, goals and interventions related to speech therapy are documented in the participant’s record, communicated in a timely manner and evaluated at appropriate intervals.
  • Provides treatment to participants in their home setting as needed.
  • Evaluates needs of EOL/AD participants; provides education, instruction to participant and/or family, consistent with their plan of care. 
  • Provides education to participants and caregivers to allow them to carry out therapeutic activities at home when indicated.
  • Participates in rehabilitation services related data collection and quality surveillance as required.
  • Documents all direct care encounters in the participants’ record to facilitate submission of claims and diagnostic data to FCHP systems.
  • Attends regular staff meetings and other meetings as assigned.
  • Performs all other duties as assigned.

Qualifications

Education:

  • Graduate of an accredited Speech Language Pathology program accredited by ASHA.

License/Certifications:

  • Licensed as a Speech Language pathologist in Massachusetts.  Certificate of Clinical Competence. 
  • CPR certification or willingness to obtain certification within 60 days of hire.
  • Willingness to occasionally assist other SE PACE centers either in person or remotely, as appropriate, when there is an opening or a gap in coverage
  • Driving your personal motor vehicle is an essential job function of this position and the following requirements apply:
    • Must possess a valid drivers’ license
    • Must attest to no disqualifiers per Driver Safety Policy
    • Must possess and provide proof of minimal state required auto insurance
    • Must have reliable transportation

Experience: 

  • At least one year experience working with a frail or elderly population in a health care setting.

Pay Range Disclosure:In accordance with the Massachusetts Wage Transparency Act, the pay for this position is $51.74 per hour which reflects what we reasonably and in good faith expect to pay at the time of posting. Final compensation will depend on the candidate’s experience, skills, and fit with the role’s responsibilities.
