Description

JOB SUMMARY: The Family Nurse Practitioner (FNP) participates in the delivery of excellent medical services in a patient centered medical home environment with an emphasis on the prevention of disease; the nurse practitioner provides quality and affordable healthcare to all patients regardless of race, gender, age, background, religion, socio-economic status, etc. The FNP diagnoses, treats, and follows-up with all patients according to their individual medical needs. The FNP seeks out alternative methods of treatment when cost is an issue; prescribes necessary medication(s) and requests appropriate referrals when needed. The FNP also participates in various committee meetings to include Peer Review Meetings and Quality Assurance/Quality Improvement (QA/QI) Committee Meetings to coordinate improvement in all areas of the clinic with an emphasis on the importance of the individual patient and putting their needs first.


Work Schedule: Monday, Tuesday, Wednesday, and Saturday


DUTIES AND RESPONSIBILITIES:


Administrative Duties:

  • Promotes and demonstrates Hope values of: Gospel motivation, Prayer, Excellence, Stewardship, Patience, Thankfulness, Humility, Service, Dignity through Relationship and Hospitality and Joy
  • Serves on the Patient Care Team
  • Diagnoses, treats, and helps patients manage acute and chronic illnesses
  • Conducts physical examinations and interprets medical history
  • Orders and performs diagnostic tests and procedures
  • Prescribes age-specific physical therapy and rehabilitation
  • Immunizes patients against communicable diseases
  • Prescribes pharmacologic treatments (depending on state regulation)
  • Provides primary and specialty care services for adults and children
  • Refers patients to medical specialists for consultations when necessary
  • Performs minor procedures and injections
  • Counsels and educates patients about preventive measures to avoid illness
  • Maintains safe, secure, and healthy work environment by establishing and following standards and procedures; complies with legal regulations
  • Prepares records and reports as required in support of services rendered in accordance with established procedures
  • Attends and participates in patient care reports, patient care conferences, team conferences, professional staff conferences and other appropriate professional activities when necessary
  • Updates job knowledge by participating in educational opportunities; reading professional publications, and completing CME courses during the year
  • Participates in, and provides insight into the development of medical protocol
  • Participates in quality assurance/quality improvement (QA/QI) meetings and projects
  • Actively searches for ways to improve patient customer service, wait time, patient flow, utilization of clinic resources
  • Maintains all BLS certifications and state licensure. Must be certified through the American Heart Association (AHA)
  • Attends and actively participates in staff meetings
  • Works with all team members to improve clinic operations; strives to maintain positive relationships with co-workers and other      supervisory/administrative staff members
  • Assists with coverage of clinical tasks assigned to providers who are on PTO or medical leave as coordinated by the Medical Director
  • Performs all other duties as assigned

  • Always maintains the confidentiality of Hope Christian Health Center finances and patient-related information.
  • Performs all other duties as assigned. 
  • Uphold Complies with HIPAA and protects patient information.
  • The Family Nurse Practitioner ensures compliance with all 19 Health Resources and Services Administration (HRSA) Health Center Program Requirements.
  • The Family Nurse Practitioner is also expected to actively participate in and support quality improvement activities and the clinical practice team efforts emphasizing the importance of the individual patient and putting the patient’s needs first.

Requirements

Qualifications:

  • Maintains all BLS certifications and state licensure. 


Education Required:

  • Master’s degree from an accredited college or university. 
  • Successful completion of a Family Nurse Practitioner program from an accredited program or school.
  • Must be licensed in the State of Nevada.

  • Ability to maintain confidentiality of information (patient’s personal, medical, financial information).
  • Ability to correctly calculate and administer doses of medication.


TYPICAL PHYSICAL DEMANDS: 

Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, pulling and lifting and/or moving up to 25 pounds. Frequent sitting. Manual dexterity. Good reading eyesight and vision for close work. Ability to communicate by speaking and hearing in person or on the phone. Ability to travel to various locations.
