Home Health & Hospice Client Care Coordinator

Location: North Platte, NE

Brookestone Home Health & Hospice, a member of the Vetter Senior Living family, is seeking a compassionate and organized Client Care Coordinator to join our team. We provide personalized home health and hospice services that allow patients to receive exceptional care in the comfort of their own homes.

The Client Care Coordinator works closely with referral sources, patients, families, and our interdisciplinary team to coordinate admissions, manage referrals, schedule services, and ensure a smooth transition into care. Strong communication, organizational, and customer service skills are essential.

Qualifications:

  • Must hold a current Nebraska license recognized by the Nebraska Department of Health and Human Services (DHHS) as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), Speech-Language Pathologist (SLP), or Medical Social Worker (MSW).
  • Healthcare, home health, or hospice experience preferred.
  • Excellent communication, organizational, and customer service skills.
  • Ability to multitask in a fast-paced environment.
  • Compassionate, professional, and team-oriented.
