Stonington Institute is committed toĀ providing quality substance abuse treatment options for adults in Southeastern Connecticut and surrounding areas.  We offer medical detox as well as residential addictions treatment for active duty and retiredĀ military service members in ourĀ North Stonington location.  Our clinic in Groton, CT, offers partial hospitalization and intensive outpatient treatment, and is convenientlyĀ locatedĀ across the street from The Inn at Trails Corner, a new 180 bed dormitory-style sober community for adult men and women who attend our outpatient day treatment programs. The Inn offers overnightĀ accommodations, full-meal service, indoor/outdoor recreation options, a workout room, andĀ computerĀ center for all clients.Ā We accept private insurance, state-fundedĀ plansĀ and self-pay arrangements.Ā Ā

Essential Job Functions:Ā 

  • Completes a Discharge Plan for each client admitted to the Detox Program, documenting progressĀ dailyĀ on approved forms and assuring completion of Discharge Checklists, ā€œRisk of Overdoseā€ Form, Basis 32, and Satisfaction Survey for each client.Ā Ā Ā Ā 
  • AssuresĀ appropriate ROIsĀ are completed for referral agencies and others asĀ required.Ā 
  • Ensure the Step-Down process isĀ maintainedĀ to provide continuity of care.Ā Ā Ā 
  • Works collaboratively with clients and members of the treatment team to incorporateĀ client needs and preferences and the recommendations of the multidisciplinary treatment team into the discharge planning process.Ā Ā Ā Ā Ā Ā Ā Ā Ā Ā Ā 
  • Acts asĀ a liaisonĀ with community providers and respond to community inquiries related to discharge planning processes and procedures.Ā

Qualifications

Bachelor’s degree required. Three (3) years’ experience in the substance abuse/mental health treatment field preferred. Previous case management/discharge planner experience required.Ā
