Community Services Direct Support ProfessionalÂ
Location: Norwich, NY
Schedule: Part-time, up to 29 hours per week
Pay: $17.50- $18.75

The Community Services Direct Support Professional is responsible for providing direct care through implementation of Habilitation Plans. Â Specific duties and responsibilities include, but are not limited to:
- Provide supervision and all direct care required to maintain safety and comfort of the person receiving services
- Transport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and explorationÂ
- Development and planning of inclusive community activities
- Establish and maintain positive relationships with area businessesÂ
- Assist people receiving services with completing daily or routine domestic or personal care tasks
- Foster and promote skills leading to independent living
- Assist with the development of Habilitation Plans and following these plans for training and assistanceÂ
- Provide training and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required
- Provide supervision to people supported during program hours and during community outings as to ensure their health and safety
- Complete required documentation in conformance with rules, regulations, policies and procedures, and billing practices

Requirements
- High School Graduate/GED with some knowledge of or experience working with developmentally disabled preferred.
- Possess a valid driver’s license acceptable to the agency’s insurance carrierÂ
