We are currently seeking a Discharge Planner for Residential Services for Riveredge Hospital.  Simply apply to the posting and discuss your interests with the hiring managers if you are selected to come in for an interview.

The Discharge Planner is responsible for the coordination of the residents’ case management and discharge needs. This includes assisting the treatment team in the development of appropriate discharge plans, the facilitation of the attainment of resources to meet this plan and the education of the residents regarding the discharge process. The discharge planner is a liaison between the treatment team and community. 

Experience:

One (1) year of related experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.

Bachelor Degree in psychology, social work, or related field preferred.

Additional Requirements:

Successful completion of CPR and CPI that is provided during orientation. May be required to work overtime and flexible hours.
