We are currently seeking a Discharge Planner for Residential Services for Riveredge Hospital. Ā Simply apply to the posting and discuss your interests with the hiring managers if you are selected to come in for an interview.

The Discharge PlannerĀ is responsible forĀ the coordination of the residents’ case management and discharge needs. This includesĀ assistingĀ the treatment team in the development ofĀ appropriate dischargeĀ plans,Ā the facilitationĀ of the attainment of resources to meet thisĀ planĀ and the education of the residentsĀ regardingĀ the discharge process. The discharge planner is a liaison between the treatment team andĀ community.Ā

Qualifications

Experience:

One (1) year of related experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.

Bachelor Degree in psychology, social work, or related field preferred.

Additional Requirements:

Successful completion of CPR and CPI that is provided during orientation. May be required to work overtime and flexible hours.
