Position Overview
Under the supervision of the Chief Growth Officer, The Director of Community Impact Programs is responsible for designing, launching, and leading a cross-referral and service-coordination program that connects MedZed with community-based organizations, health systems, and county and municipal social service agencies to improve care for California Enhanced Care Management (ECM) and Community Supports (CS) members. We are seeking the Director of Community Impact Programs to be part of the Business Development Team, playing a key role in business development and strategic growth, building relationships that expand MedZed’s ECM and CS membership panels through referrals, partnerships, and service integration. You will be combining community systems expertise with partnership-building and growth strategy experience, ensuring that MedZed delivers high-impact care while strengthening its presence across California.
Key Responsibilities
Program Design & Leadership
- Establish and operationalize a cross-referral program connecting MedZed with community-based organizations (CBOs), health care providers, housing/homelessness services, behavioral health agencies, and county/municipal entities.
- Develop referral protocols, communication workflows, and technology-enabled processes to ensure seamless member transitions across organizations.
- Create infrastructure in partnership with MedZed IT teams to track referrals, outcomes, partner utilization, and member engagement patterns.
Business Development & Growth
- Fully own referral growth, set quarterly targets for new partner activation and member enrollment in collaboration with the Growth and Operations teams.
- Identify, cultivate, and manage partnerships that generate new ECM and CS referrals and expand MedZed’s membership panels.
- Serve as a key representative in meetings with health plans, county agencies, provider networks, and CBO leaders to raise awareness of MedZed programs and value.
- Design outreach and engagement strategies that increase cross-referrals from community partners, hospitals, social service agencies, and other potential referral sources.
- Support proposal development, partnership agreements, and contracting discussions that align with MedZed’s growth goals.
- Monitor referral pipelines, analyze trends, and collaborate with growth and operations teams to adjust strategies and ensure targets are met.
Partnership Development & Ecosystem Integration
- Build and maintain collaborative relationships across the CalAIM ecosystem, including social services, public health, behavioral health, housing, justice-involved services, and immigrant/refugee support networks.
- Negotiate MOUs, shared protocols, and data-sharing agreements that support both service integration and referral growth.
- Represent MedZed in regional forums and CalAIM-related collaboratives to strengthen visibility and trust across stakeholders.
Operational Oversight & Team Management
- Lead a team responsible for community partnerships, referral coordination, and navigation support.
- Develop KPIs for referral volume, service integration, partner engagement, and member outcomes.
- Ensure compliance with CalAIM, Medi-Cal, HIPAA, and health plan contractual standards.
- Collaborate closely with MedZed care teams to optimize workflows, reduce friction between partners, and strengthen member support across medical, behavioral, and social domains.
Qualifications
- Bachelor’s degree in public health, Social Work, Public Administration, Business, Health Policy, or related field.
- 5+ years of experience in community health, care coordination, population health, or social services—with at least 3 years involving partnership development or business development.
- Deep knowledge of Medi-Cal, CalAIM, ECM, Community Supports, and county-level service systems in California.
- Demonstrated success developing partnerships that drive program growth, referrals, or revenue.
- Strong leadership experience with multidisciplinary teams.
- Exceptional relationship-building, communication, and negotiation skills.
- Experience with referral platforms, care management systems, and data-sharing tools.
- Experience in managed care, ECM provider networks, or health plan partnerships preferred.
- Familiarity with county-level processes in homelessness services, public health, behavioral health, and social services preferred.
- Prior role focused on network development, outreach, community engagement, or growth strategy preferred.
- Bilingual/bicultural experience.

Working Conditions
- Hybrid remote role with frequent travel across California for partner meetings, conferences, and community engagement activities.
- Occasional evening/weekend events depending on partner schedules.
Physical Requirements
- This position requires the employee to remain in a stationary position for prolonged periods while performing desk-based computer tasks.
- The employee must be able to constantly operate a computer, keyboard, and other office productivity machinery, such as a calculator, copy machine, and computer printer.
- The person in this position may need to occasionally move about inside the office/remote workspace to access file cabinets, office machinery, or other resources.
- The employee may occasionally be required to lift and move objects weighing up to 10 pounds (e.g., a laptop, files, or small office equipment).
- Clarity of vision at approximately 20 inches or less is required for working with computers and reading small print.
