Responsibilities

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

 

In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantage members who reside in participating long term care facilities. You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives and you will act as clinical resource to the participating providers and facilities.
  • Provide and/or coordinate primary clinical care services for assigned Nursing Home Residents
  • Acts as resource and contact for member/designated representative regarding all aspects of Advantagemember care plan
  • Acts as team member and clinical resource to participating providers/facilities
  • Acts as resource and contact for member/designated representative regarding all aspects of memberindividualized care plan and disease process

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program required.
  • NYS RN/NP or PA License, Registration, DEA certificates.
  • One to two years clinical experience in role of nurse practitioner or physician assistant required including 1 year in a geriatric specialty.
  • Previous clinical experience in acute, subacute and/or long term care setting preferred.
  • Prior experience in managed care a plus.
  • Proficiency in computer skills, electronic medical record, case management software, word, excel required.
