Canyon Home Health is looking for a Physical Therapist (full-time or part-time) who wants real flexibility, real autonomy, and a caseload that lets you actually connect with patients — one home visit at a time. QualificationsAs our PT, you’ll evaluate patients and build individualized care plans, then adjust them as progress unfolds. You’ll perform skilled therapy in patients’ homes, keep physicians in the loop on progress and changes, and guide patients and families through home exercise programs. You’ll also supervise PTAs and HHAs as needed, stay current on documentation (OASIS, progress notes, discharge summaries), and bring sound clinical judgment to every visit. A current CPR certification and a collaborative, team-first mindset round out the role.
