As a Social Worker with Fresenius Medical Care, you will provide psychosocial services for our dialysis clinic patients. You will work with the health care team to promote positive adjustment, rehabilitation, and improved quality of life for our patients. Support the clinic staff in understanding the emotional, psychological, and behavioral impact of chronic kidney disease on the patient and family. Assess patients' psychosocial status, strengths, and areas of need affecting rehabilitation and treatment outcomes. Collaborate with patients and health care team to identify effective interventions to meet rehabilitation and treatment goals. Utilize patient education programs, social work theory, methods, and interventions to address barriers and meet treatment goals. Provide monitoring and interventions for patient adjustment to dialysis and psychosocial status. Assess patient knowledge of kidney disease for barriers to treatment adherence. Provide supportive counseling within clinical training and state license scope. Assist with referrals to community resources to facilitate treatment outcomes. Maintain knowledge of vocational/educational rehabilitation programs and assist patients with access. Assess patient awareness of advance directives and facilitate discussions as needed. Participate in patient DNR status discussions. Report on quality indicators related to adherence and quality of life. Provide education tailored to patient learning styles and communication barriers. Provide counseling to patients seeking transplant and facilitate transplant referral process. Educate patients/families on psychosocial issues related to ESRD and support services. Review patient rights, grievance information, and facility policies with patients. Provide psychosocial training to clinic staff as needed.
