About the Role:
Som Medical Practice is looking for a passionate and skilled Nurse Practitioner to join our growing team in Niagara Falls, Olean, Lockport, and Hamburg. This is an exciting opportunity to make a meaningful impact on patient lives in a supportive and collaborative clinical environment. Responsibilities:
- Conduct comprehensive patient assessments, including physical exams, medical histories, and diagnostic evaluations
- Diagnose and treat acute and chronic illnesses in alignment with evidence-based clinical guidelines
- Develop and manage individualized patient care plans and treatment protocols
- Order, interpret, and follow up on laboratory tests, imaging, and other diagnostic studies
- Prescribe medications and provide patient education on medication management
- Collaborate with physicians, specialists, and care team members to ensure continuity of care
- Maintain accurate and timely documentation in the electronic health record (EHR) system
Requirements:
- Active Nurse Practitioner (NP) license in the state of New York
- Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
- Current board certification (FNP-C, ANP-C, or relevant specialty certification preferred)
- Valid DEA license and prescriptive authority in New York State
- Experience in a primary care, internal medicine, or outpatient clinical setting preferred
- Proficiency with electronic health record (EHR) systems
- Strong interpersonal and communication skills with a patient-centered approach
