Nurse Navigator - HIMG-Oncology (Full Time) - 7317

St. Mary's Medical Center is seeking a full time Nurse Navigator for our HIMG - Oncology department.  The Oncology Navigator will serve as a dedicated patient advocate and care coordinator who guides cancer patients through their treatment journey, from diagnosis to survivorship or end-of-life care.  This role will:

• Provide individualized education and emotional support
• Coordinate multidisciplinary care and appointments
• Identify and address barriers to care
• connect patients wiht community and financial resources
• Facilitate communication between patients, families, and care teams
• Track patients throughout their care continuum to prevent care gaps.

Education:  Current RN license in WV, or compact state.  BSN preferred.

Experience:  2-5 years position related experience.  Preference will be given to those experienced in adult oncology either through working on an IP dedicated oncology unit or OP Cancer Center.

Certifications/Skills:  BLS required.  OCN or ONN certification preferred.  Microsoft Office experience required.
