Description

Summary

This position is established for the purpose of providing case management to PASSPORT and Assisted Living Waiver individuals.


Essential Functions

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  1. Conducts timely phone calls and in-person contacts with individuals in accordance with program specifications to monitor the delivery of all services in the individual’s service plan.
  2. Assists individuals in gaining access to any necessary services regardless of funding source which enable individuals to remain in their community living arrangement.
  3. Accurately and timely authorizes changes in amount, scope, and duration of waiver services.
  4. Accurately coordinates provision of waiver services with other services in the individual’s service plan to prevent service duplication.
  5. Maintains ongoing contact with service providers to monitor service delivery and resolve service provision problems.
  6. Maintains contact with informal caregivers to obtain necessary information and coordinate formal and informal services.
  7. Conducts timely Level of Care and Comprehensive assessments as needed, according to program specifications.
  8. Accurately maintains all appropriate documentation in formats and time frames prescribed by the PAA and the Ohio Dept. of Aging.
  9. Maintains job functions as provided by and unique to this distinct work unit, separate from other units and reporting up a separate supervisory chain.  
  10. Understands the Agency’s Conflict of Interest Policy and complies with the Ohio Aging Network’s requirements for disclosure.
  11. Participates in training to maintain and improve job skills.
  12. Assists with and completes special projects and activities as assigned.
  13. Attends Agency meetings and trainings as assigned including but not limited to team meetings, division meetings, all staff meetings, HIPAA trainings, and other.
  14. Utilizes electronic tools and software to complete tasks. 
  15. Performs duties with respectful communications & behaviors.


Requirements

  1. Ability to relate well with all individuals, community agencies, and Area Agency staff.
  2. Strong verbal and written communication skills.
  3. Knowledgeable in Microsoft Word and basic computer skills.
  4. Successful completion of the Ohio Dept. of Aging certification required within 6 months of employment.
  5. Ability to communicate professionally, respectfully, and effectively.
  6. Ability to make sound decisions and demonstrate critical thinking skills.
  7. Ability to prioritize, organize and maintain accuracy in a flexible job environment.

Carry job related items weighing up to 30 pounds into private homes and facilities which may not be readily accessible.

HIPAA/Confidentiality

Must maintain strict confidentiality and comply with all Agency HIPAA policies and procedures.


Travel

Ability to travel in a multi-county service area and access private homes and facilities, which may not be readily accessible.


Eligibility Requirements

Registered Nurse or Licensed Social Worker holding an active license to practice in the State of Ohio.
