Concierge Home Care is hiring a Behavioral Health Registered Nurse (RN) to support our Sebring territory.

Territories are designed to minimize drive time with clustered scheduling.

Your Role as a Behavioral Health RN
- Provide skilled nursing care with a mix of focused visits on mental and behavioral healthÂ
- Perform patient assessments and develop individualized care plansÂ
- Support patients in maintaining safety and independence at homeÂ
- Collaborate with physicians and interdisciplinary teamsÂ
- Monitor emotional and physical status and adjust care plans as neededÂ
- Communicate updates on patient condition to the care teamÂ

Qualifications
Required
- Active, unencumbered Florida RN licenseÂ
- Valid driver’s license, reliable transportation, and auto insuranceÂ
Behavioral Health / Psychiatric RN Criteria (Medicare-Compliant)
To qualify for this role, candidates must meet ONE of the following:
- Master’s Degree (Psych/Mental Health Nursing)Â
- Licensed RN with a Master’s specialization in psychiatric/mental health nursingÂ
- Recent experience (within ~3 years preferred) in:Â
- Psychiatric hospitalÂ
- Psychiatric home healthÂ
- Partial hospitalization program (PHP)Â
- Outpatient psychiatric settingÂ
- Bachelor’s Degree in Nursing (BSN)Â
- Licensed RN with at least 1 year of recent psychiatric nursing experience (within ~3 years preferred) in:Â
- Acute psychiatric careÂ
- Psychiatric home careÂ
- PHP or outpatient psych servicesÂ
- Licensed RN with at least 1 year of recent psychiatric nursing experience (within ~3 years preferred) in:Â
- Associate Degree or Diploma in NursingÂ
- Licensed RN with at least 2 years of recent psychiatric nursing experience (within ~3 years preferred) in:Â
- Acute psychiatric careÂ
- Psychiatric home careÂ
- PHP or outpatient psych servicesÂ
- Licensed RN with at least 2 years of recent psychiatric nursing experience (within ~3 years preferred) in:Â
Additional Preferred Experience
- Medical/Surgical nursing experience strongly preferredÂ
- Experience with geriatric behavioral health patientsÂ
- Ability to work independently while collaborating with a clinical teamÂ
Clinical Compliance & Scope
- Services are provided under a physician-established and reviewed Plan of Care (POC)Â
- Role supports delivery of psychiatric evaluation and nursing interventions for home health patientsÂ
- Must meet qualifications aligned with Medicare home health psychiatric nursing guidelinesÂ
