Job DetailsJob Location: Coastal Health Corporate - Orange Park, FL 32073Quality Care Coordinator Location: Corporate Office Department: Quality Company: Coastal Health Position Summary Coastal Health is seeking a detail-oriented and compassionate Quality Care Coordinator to join our Quality Department at our Corporate Office. This role is responsible for ensuring patients receive appropriate follow-up care after emergency room visits, hospital discharges, and other care transitions. The Quality Care Coordinator plays a vital role in supporting quality outcomes, reducing readmissions, improving patient engagement, and ensuring continuity of care. Essential Duties and Responsibilities Monitor daily reports related to emergency room visits, hospital admissions, and discharges. Contact patients following emergency room visits and hospitalizations to ensure appropriate follow-up care is scheduled and completed. Coordinate with primary care providers, specialists, and clinical staff to facilitate timely patient appointments. Document all patient outreach and care coordination activities accurately and timely. Track quality measures and identify patients requiring intervention to meet organizational quality goals. Assist with closing care gaps and ensuring compliance with quality initiatives and regulatory requirements. Collaborate with providers, care teams, and leadership to improve patient outcomes. Maintain confidentiality and compliance with HIPAA regulations. Generate reports and provide updates on patient follow-up activities and quality metrics. Qualifications High school diploma or equivalent required; Associate's or Bachelor's degree in Healthcare Administration, Nursing, Public Health, or a related field preferred. Minimum of 1-2 years of experience in healthcare, care coordination, quality improvement, case management, or a related role preferred. Strong communication and customer service skills. Experience working with electronic health records (EHRs) and healthcare reporting systems. Excellent organizational skills and attention to detail. Ability to manage multiple priorities in a fast-paced environment. Knowledge of healthcare quality measures, value-based care, and population health initiatives is preferred.
