Job DetailsJob Location: Orange, VA 22960POSITION OVERVIEW: Serves as a liaison and advocate for residents, supporting their social and emotional well-being. Assists with planning, organizing, implementing, and evaluating the Social Services program to ensure each resident’s psychosocial needs are identified, addressed, and maintained on an individual basis. Carries out all responsibilities in accordance with applicable federal, state, and local regulations, as well as facility policies and procedures. ESSENTIAL FUNCTIONS: Assist in planning, developing, organizing, implementing, and evaluating the operation of the Social Services Department. Consult with medical and nursing services, consultants, and other departments and assist in implementing changes as needed. Obtain residents’ social histories and evaluate social and family information, assist in determining plans for social treatment and interpreting residents’ social, psychological, and emotional needs, and document and communicate same as appropriate to the interdisciplinary team. Participate in planning, development, and implementation of social care plans, resident assessments, and discharge planning to meet the needs of the residents, their families, and the facility. Assist in coordinating the social services programs with residents, their families, staff members, and community agencies through interviews, consultations, and direct involvement in the programs. Provide information to residents and their families regarding available support agencies and financial assistance programs to meet the needs of the residents. Assist in developing a preliminary and comprehensive assessment and written care plan that identifies the emotional and social needs of the resident and the goals to be accomplished for each need identified. Encourage residents to participate in the development and review of care plans. Participate in assessing, reviewing, and revising care plans as required. Assist in ensuring that quality control measures are continually maintained. Develop and maintain rapport with all services to ensure that a team effort is achieved in developing a comprehensive care plan for all residents. Maintain resident confidentiality and treat residents with kindness, dignity, and respect. Know and comply with Residents’ Rights rules. Review, promptly report to the Director of Community and Customer Relations, and maintain written records of all resident complaints and grievances. Delegate administrative authority, responsibility, and accountability to other social services personnel as deemed necessary to perform assigned duties to cover for absence or scheduled vacation. Attend and participate in facility sponsored events and special functions as deemed appropriate by facility Administrator. ADDITIONAL FUNCTIONS: Maintain professional competence through participation in continuing education programs, seminars, and training programs. Attend and participate in all required training programs upon hire and annually or as assigned thereafter. Recommend to the Department Director the equipment and supply needs of the Department and assist in the ordering and purchasing of equipment and supplies as necessary. Forecast the needs of the department, assist in preparing and planning the department’s budget for equipment, supplies, and labor, and submit recommendations to the Director of Community and Customer Relations. Other duties as assigned. QualificationsKNOWLEDGE, SKILLS, AND ABILITIES: Ability to react calmly and professionally in complex or difficult situations. Ability to prioritize tasks, make decisions, delegate duties, and manage multiple objectives, projects, and deadlines while maintaining a professional demeanor. Ability to communicate clearly and effectively, both verbally and in writing, with residents/patients, staff, family members, vendors, suppliers, and other contacts, both inside and outside the facility. Must exhibit patience and a positive attitude towards the elderly population and a desire to provide a service to the community. EDUCATION AND EXPERIENCE: Bachelor’s or equivalent degree in Social Work or Human Services preferred. Three to five (3-5) years’ experience in Social Work in a long-term care or similar facility. PHYSICAL REQUIREMENTS: Ability to stand, walk, sit, stoop, bend, and reach for extended periods. Ability to lift up to 20 pounds constantly (up to 100% of the time spent in performance of duties). Ability to determine by visual inspection and by smell if a hazardous condition exists. SPECIAL REQUIREMENTS: Requires a satisfactory general background check, including state and local criminal history checks, as well as a satisfactory drug screening.
