Job DetailsJob Location: Orange, VA 22960Position Type: Part TimeSalary Range: $32.00POSITION SUMMARY  Reports to the Director of Nursing and supervises the day-to-day functions and activities of a designated unit within the facility.      All duties are performed in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility.  ESSENTIAL FUNCTIONS  Supports and participates in developing a cohesive nursing team throughout the facility by leading, motivating and supporting staff in daily operation.  Supports a positive and professional image through leadership activities that emulate the organization’s mission statement and encourages a spirit of cooperation and teamwork.  Ensures that a sufficient number of nursing personnel and/or Certified Nurse Aides (CNAs) are available for each shift on their unit to ensure that quality care is maintained. Communicates staffing needs to the supervisor/scheduling coordinator.  Supervises and directs nursing care on the assigned unit(s). Initiates and executes constructive discipline and performance evaluations in accordance with established policies and procedures.   Ensures corrective action has been completed on medical record audits and consultant reports.  Completes routine rounds during shift to assess residents and to determine if their needs are being met in accordance with standards of care, resident requests, and resident rights and ensure nursing service personnel are performing their work assignments in accordance with acceptable nursing standards at all times.  Implements, coordinates and supervises compliance with resident care programs on each unit, such as: Incident & Accident, Infection Control, Wound Care, Coumadin, etc.  Ensure that all admissions and re-admissions are processed in accordance with federal, state, local and facility guidelines and policies. Review, utilize and complete the Admissions Checklist.  Reviews unit 24 hour report daily and supervises/coordinates nursing services and care plan interventions.  Reviews nurses’ notes as directed to ensure they are informative and descriptive of the nursing care being provided, that they reflect the residents’ response to the care provided, and that such care provided is in accordance with resident wishes.  Addresses clinical issues, goes on rounds, and obtains MD orders as necessary. Follows up, notates and posts MD orders and ensure follow through and completion.  Participates in the development of a written plan of care (Initial, acute and comprehensive) for each resident from the MD orders that identify the problem/need of the resident; indicate the care to be given, goals to be accomplished and which professional service is responsible for each element of care. Reviews individual care plans to ensure for appropriate resident problem statements, goals, interventions/approaches, and revisions based upon resident needs.  Ensures that all personnel involved in providing care to the residents are aware of the resident’s care plan. Ensure that nursing personnel refer to the plan of care prior to administering daily care to the residents.  Serves as the point person for the residents/family on their assigned unit. Addresses any concerns, complaints, grievances the resident or their family may have; or elevate them to the appropriate level.  Ensures that CNA documentation is accurate and complete; providing 1:1 education as appropriate.  Ensures that all Licensed Nurses comply with written procedures for the administration, storage and control of medications (including narcotics) and supplies.  Observes medication pass, treatment schedule and corresponding documentation to ensure compliance with applicable regulations and policies.  Provides nursing support on the units and directs care as necessary. Assists Charge Nurse in assessment of resident conditions and notifications as necessary. Provides direct care as necessary to cover absences, vacations, or other staffing challenges.  Participates in the on-call rotations as directed.  Performs administrative duties such as completing medical reports and forms, evaluations, studies, charting, etc., as necessary and directed.  Inspects the nursing service areas daily (stations, med rooms, supply rooms, etc.) to ensure that they are maintained in a clean, sanitary, organized and safe manner.  Attends mandatory in-services and facility/department meetings as required.  Ensures that all OSHA and safety standards are followed in accordance with federal and state regulations, and facility policies and procedures. Maintains safe and clean work area and adheres to safety standards.  Complies with, and ensures that facility staff complies with all Infection Control, Universal Precautions and OSHA Standards for the healthcare professional in accordance with federal, state, local and facility regulations, policies and procedures.  Adheres to and ensures compliance to established HIPAA confidentiality standards of resident and client location information.  Actively develop and maintain good working relationships with all nursing facility staff and set an example of positive feeling, calm and professionalism for staff members, residents and family members.  Other related duties as needed or assigned.  MARGINAL FUNCTIONS  Assist administrative nursing staff in providing information to staff regarding resident rights, facility policies, and changes to state, federal, OSHA, CDC, and other regulations as needed or requested.  Assist in the administration of and reading of the annual TB tests, Flu shots, and other injections or laboratory tests as needed or requested.  Assist with monthly chart audits, MARS, and TARS as assigned.  Participates as directed in the recruitment, interview and hire of nursing personnel for the unit.  Attend and actively participate in charge nurse and CNA meetings.  Attend and participate in continuing education programs as identified or required in order to maintain professional competence and comply with federal, state, local or facility policies and regulations.  Attend and actively participate in meetings, committees, or workgroups as needed or as directed.  QualificationsQUALIFICATIONS  Education/Certifications:   Current, un-restricted, valid licensure as a Registered Nurse or Licensed Practical Nurse issued by the Commonwealth of Virginia Department of Health Professions.  Current CPR certification (must be obtained within 30 days of hire if not already certified).  Experience:  Three to five (3-5) years nursing experience, preferably in a long-term care facility.  Six (6) months previous supervisory experience preferred.  Must be familiar with Microsoft Office software, including Word, Excel, and calendar programs.  Skills:  Ability to communicate effectively and clearly (both verbally and in writing) with staff, co-workers, administration, residents, family members, and contacts at all levels; both inside and outside of the facility.  Ability to react calmly and professionally in complex or difficult situations.  Ability to prioritize tasks, make decisions, delegate duties, and manage multiple objectives, projects, and deadlines while maintaining a professional demeanor.  Must exhibit patience and a positive attitude towards the elderly population and a desire to provide a service to the community.  PHYSICAL DEMANDS AND REQUIREMENTS  Ability to stand, walk, sit, stoop, bend, and reach for extended periods of time.  Ability to lift up to 20 pounds frequently and up to 35 pounds occasionally in the performance of duties.  Manual dexterity that allows grasping, carrying and holding equipment and supplies or for resident care as needed to perform tasks.  Ability to see, hear, and verbally communicate in the performance of duties.  DISCLAIMER  The above listing is a brief summary of the nature and scope of duties and is not intended to be a comprehensive listing of all tasks and responsibilities of the position described.  The Facility Administration reserves the right to add, change, delete, or otherwise modify this description, including Essential Functions, at its sole discretion without further permission or authority required.
