Job Description Summary

The Community Social Worker – Mobile Health Unit plays a key role in addressing the social determinants of health for patients served in the mobile healthcare setting. This position provides on-site support, case management, and outreach to individuals and families who may experience barriers to accessing traditional healthcare services. The Community Social Worker collaborates with the mobile health clinical team, community organizations, and social service agencies to ensure patients receive comprehensive, holistic care that extends beyond medical treatment.

Job Description

Key Responsibilities

Client Support & Case Management – 45%

  • Conduct assessments of patients’ social, behavioral, and resource needs during mobile health visits.

  • Develop and implement individualized service and care coordination plans.

  • Provide crisis intervention, brief counseling, and advocacy.

  • Assist patients in navigating healthcare, housing, food security, behavioral health, and financial assistance resources.

  • Ensure continuity of care through regular follow-up and coordination with providers and community agencies.

Community Outreach & Patient Navigation – 30%

  • Engage directly with patients in community settings served by the mobile health unit.

  • Act as a liaison between patients, healthcare providers, and community resources.

  • Participate in outreach events and resource fairs to expand community awareness of mobile services.

  • Educate patients and families on available support programs and benefits eligibility.

  • Advocate for patients to remove barriers related to transportation, insurance, or access to care.

Documentation & Compliance – 15%

  • Maintain accurate, confidential client records in accordance with HIPAA, legal, and ethical standards.

  • Document assessments, service plans, interventions, and outcomes in the electronic health record (EHR).

  • Provide data for program evaluation, grant reporting, and quality improvement initiatives.

Team Collaboration & Professional Development – 10%

  • Work closely with mobile health providers, nurses, medical assistants, and drivers to ensure integrated patient care.

  • Participate in case conferences and interdisciplinary care planning.

  • Remain current with best practices, community resources, and policy changes impacting underserved populations.

  • Engage in ongoing training and professional development.

Qualifications

  • Bachelor’s degree in public/community/social work

 

OR

 

  • Associate degree in public/community health or healthcare related field with a minimum of one (1) year of experience in a directly related field or working in a community-based setting including paid or volunteer activity with a non-profit organization, church, healthcare organization, neighborhood community center, or other relevant community-based involvement.

  • Completion of SCCHW Certification within (1) year of hire: https://www.scchwa.org/credentialing-information-learners and/or other Community Health Worker training course.

  • Valid driver’s license, an acceptable 3-year motor vehicle as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto-insurance.

Additional Job Description

Education: Bachelor's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: BSW licensure by the South Carolina Board of Social Work Examiners.  Certification in case management preferred.
