SUMMARY OF POSITION FUNCTION: 

Provides professional nursing support and coordination of behavioral healthcare services for individuals served while adhering to agency policies, procedures, and core values. Utilizes clinical knowledge, documentation standards, and agency technology systems to support quality care, staff training, service coordination, and compliance with healthcare and regulatory requirements. Closely collaborates with leadership team, interdisciplinary teams and community providers to address medical needs, promote continuity of care, and support overall health and wellness outcomes.

MAJOR DUTIES AND RESPONSIBILITIES: 

  • Knowledge and demonstration of Agency core values, policies and procedures
  • Demonstrates ability to effectively use technology and appropriate use of support tools
  • Proficient in the use of Agency software such as Microsoft Office Products and Cerner Millennium.
  • Professionally performs general nursing duties and medical care as necessary in a BH/SU setting
  • Knowledge of appropriate documentation standards and execution within department guidelines
  • Responsible for conducting audits of nursing documentation and required site logs
  • Administers IMs according to provider orders and agency procedure as necessary
  • Required to work respectfully with the nursing team, pharmacies and individuals to resolve medication issues
  • Provide supervision and training to staff as instructed by Director of Nursing & Medical Services
  • Serves as resource person/consultant to outside service providers in identifying individuals for referral
  • Works cooperatively with other providers to assure linkages are made to appropriate services.
  • Assists with special projects that may involve the gathering of data, tracking and monitoring of individuals who are at risk for health issues.
  • Performs other duties as assigned.
  • Travel may be required between sites on an as needed basis

SKILLS/COMPETENCIES:

  • Experience in providing medical services such as conducting medical history reviews, obtaining basic vital signs and providing IM’s to individuals 
  • Skilled in providing education regarding medical/medication issues.
  • Skilled in use of Cerner Millennium and Microsoft Office Products
  • Demonstrates skill relating to BH & CD populations serving with kindness, respect, accuracy and timeliness.

EDUCATION REQUIREMENTS:

  • Minimum NYS licensure of licensed practical nurse (LPN)
  • Educational qualifications must match regulatory requirements to perform medical services
  • CPR/AED certification

EXPERIENCE:

  • Experience must match regulatory requirements
  • Must have experience in providing medical services to individuals with behavioral health needs
  • Minimum one year experience is LPN 
  • Must possess a valid Driver’s License with an acceptable driving record.  

COMPENSATION: $23.09/hr - $29.44/hr
