The role you’ll contribute:
The Certified Clinical Transplant Social Worker is responsible for completing comprehensive pre-transplant psychosocial evaluations on pediatric, adolescent, adult and geriatric patients. The Social Worker documents in patient charts in a timely, efficient, and clinical manner. The Social Worker determines patient's ability to manage his or her own care post-transplant/Ventricular Assistance Device (VAD) care, including transportation, support system/caregiver, financial status, housing, mental status, and functional status. The Social Worker works directly with the pharmacist and financial coordinator to ensure the patient has the financial resources to acquire needed medications post-transplant. The Social Worker actively participates in patient and family support and education through the monthly support group. Attends and participates in team meetings that are focused on patient care and treatment planning. Provides case management, crisis intervention, and counseling where clinically indicated to patients pre- and post-transplant. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Performs comprehensive psychosocial assessments, including evaluating patient’s understanding, motivation, mental status, employment, disability, insurance coverage, compliance, social history, substance use, functional status, caregiver, and transportation plan.
Collaborates with the multi-disciplinary team to establish and document patient goals and treatment plans.
Serves as a consultant and educator to the multi-disciplinary team regarding psychosocial components of illness, disability, and hospitalization.
