What You'll Do
As a Post-Acute Care Provider, you will:
- Ā Provide comprehensive primary care services in skilled nursing and post-acute care facilities.Ā
- Ā Evaluate, diagnose, and manage acute and chronic medical conditions.Ā
- Ā Collaborate with physicians providing Osteopathic Manipulative Treatment (OMT) services.Ā
- Ā Identify patients who may benefit from OMT and coordinate referrals.Ā
- Ā Develop individualized treatment plans that improve function, mobility, and quality of life.Ā
- Ā Perform routine follow-up visits, admissions, discharge planning, and transitional care.Ā
- Ā Collaborate closely with nursing staff, therapists, specialists, and facility leadership.Ā
- Ā Educate patients and families regarding disease management and preventive care.Ā
- Ā Maintain accurate and timely documentation using electronic health records.Ā
Qualifications
We're looking for providers who have:
- Ā Current Nurse Practitioner or Physician Assistant license in Florida (or ability to obtain)Ā
- Ā National Board CertificationĀ
- Ā DEA license (or ability to obtain)Ā
- Ā Excellent clinical judgment and communication skillsĀ
- Ā Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, Emergency Medicine, or Post-Acute Care preferredĀ
- Ā Skilled Nursing Facility experience is a plus, but not requiredĀ
- Ā Commitment to compassionate, patient-centered careĀ
