Title:                                        Psychiatric Mental Health Nurse Practitioner (PMHNP) 

Reports to:                             Director of Behavioral Health/Chief Medical Officer

Work Schedule:                     Full Time: Monday through Friday. Days/times may vary

 

Position Summary: 

 

Provides evidence based psychiatric evaluations and medication management for patients receiving primary health care services at Orange Blossom Family Health, including adults and school-based youth. 

Coordinates psychiatric care/referrals and collaborates with the Behavioral Health Team, Primary Care Providers, OCPS schools, and HOPE Team (Homeless Street Outreach).

Provides medical/psychiatric assessment and medication management of patients with substance use disorders (including Medication Assisted Treatment), within the scope of MOAs/MOUs in place at OBFH and collaborating agencies. 

Coordinates appropriate placement referrals as needed, including initiation of Marchman and/or Baker Acts. 

Services may be provided in varied settings at varied times, including in-office, school-based, clinical exam rooms, telehealth, off-site collaborative sites, permanent supportive housing (PSH) homes, homeless shelters, during homeless street outreach (encampments, streets, shelters, etc.). 

Ensures compliance with current federal and state standards, guidelines and regulations. Position will participate in Quality Improvement activities as needed.

 

Responsibilities & Expectations:


  • Provides evidence based psychiatric evaluations and medication management for patients receiving primary health care services at Orange Blossom Family Health, including adults and school-based youth. 
  • Conduct services both face-to-face and via telehealth. 
  • Provide brief evidence‐based psychotherapeutic interventions as needed to augment medication management. 
  • Utilize various evidenced-based, clinical assessment tools (including PHQ-9 and 9A, CFARS, GAD, MDQ, DAST, AUDIT, etc.) to determine prioiritize needs, determine appriate level of care, and track symptom improvement. 
  • Coordinate and collaborate with primary care providers, parents, caregivers, teachers, and other professionals to ensure coordinated care and improve health outcomes.
  • Facilitate referrals to other services (e.g. substance abuse treatment, specialty care and community resources) as needed.
  • Work closely with Primary Care and Behavioral Health team, including licensed providers, case managers, street outreach, and interns for case consultation, differential diagnoses,  and medication management.
  • Provide crisis intervention as needed, including to clients experiencing acute distress, suicidal thoughts, self-harming behaviors, etc. 
  • Maintain accurate and timely records of activities and services provided to each client.
  • Participate in and/or facilitate multi-disciplinary and multi-agency team meetings.
  • Work to meet patient needs effectively and resolve individual barriers through follow‐up, advocacy, and collaboration with OBFH staff and other community service providers
  • Other duties as assigned to support the mission of the organization.

 

Requirements:

 

  • Current  Florida state license as an APRN and national certification as a Psychiatric Mental Health Nurse Practitioner.
  • At least two (2) years experience in the evaluation, diagnosis, and treatment of mental health and substance use disorders. 
  • Experience working with varied populations, including youth in school-based and community settings, the homeless population, and those with substance use, and mental disorders. 
  • Experience with electronic health record documentation (Epic systems preferred). 
  • Must have strong interpersonal, written and oral communication skills. 
  • Experience utilizing Microsoft Office suite. 
  • Knowledge or Integrated Behavioral Health in Community Health (preferred)
  • Knowledge of Federal, State and County regulations for FQHCs (preferred)
  • A willingness and passion for serving disadvantaged populations
