We are seeking a motivated and compassionate Physical Therapist to join our rehabilitation team. This full-time position provides therapy services at both Dove Healthcare Osseo and Dove Healthcare South, offering the opportunity to work with a diverse resident population across two skilled nursing communities.

Key Responsibilities

Evaluate and treat residents with a variety of orthopedic, neurological, and medical conditions.
Develop individualized treatment plans to maximize mobility, function, and independence.
Collaborate with physicians, nursing staff, occupational therapy, speech therapy, and other members of the interdisciplinary team.
Complete timely and accurate documentation in accordance with regulatory and payer requirements.
Educate residents, families, and caregivers on treatment plans, safety, and home exercise programs.
Supervise Physical Therapist Assistants (PTAs) as appropriate.

Qualifications

Licensed Physical Therapist in good standing
Previous leadership or supervisory experience preferred, not required
Strong communication and organizational skills
Passion for patient-centered care and team collaboration
Ability to manage responsibilities across multiple sites
