If you are a registered nurse (RN) who desires to take the next step in the career ladder and use your experience to mentor, train, and educate your own team of caregivers, while coordinating and overseeing the care of your patients, the role of registered nurse (RN) nurse case manager may be your next calling. At Dove Healthcare, you will work with employees, patients, family members, and other professionals to coordinate all aspects of providing person-centered care.

  • Monday-Friday with an on-call rotation

Registered Nurse (RN) Nurse Case Manager Responsibilities:

  • supervise the day-to-day activities of assigned residents and employees
  • coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence
  • review and audit compliance with the patient's plan of care and changing needs
  • ensure appropriate staffing patterns are in place
  • evaluate employee duties and needs
  • hold employees accountable for following policies and procedures
  • provide a safe, comfortable, and home-like environment
  • on-call rotation
  • other duties as assigned

Required Qualifications:

  • current licensure in the State of Wisconsin as a registered nurse (RN)
  • degree from an accredited school of nursing
  • previous experience in related field
  • ability to prioritize duties in a constantly changing environment
  • effective management skills
  • customer service oriented with patients, employees, families, and other agencies
  • good problem-solving skills
  • excellent time management skills
  • ability to accept and give directions
  • work well in a team environment
