Location: Otsego Office and Remote Hours: 8-10 hours/week with opportunity for growth Competitive hourly rate $34.00- $38.00/ hour Benefits: Accrual of PTO, 401K, CEUs Essential Job Duties and Responsibilities: • Demonstrate critical thinking to advocate for patient safety, adherence to company policies, and conflict resolution. • Review data entry in medical record system to ensure accuracy for billing and compliance. • Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans as needed per manager direction. • Ensure the development and implementation of the individualized plan of care. • Assist with HHCAHPS survey analysis and implementation of quality improvement activities. • Understand and comply with policy and procedures set forth by AHC. • Monitors operations to ensure compliance with county, state and federal regulations affecting home health care. • Additional tasks, as assigned by manager. Education Requirements: Must be licensed as a Registered Nurse (RN) in Minnesota.  QualificationsAdditional Requirements: • Working knowledge of Microsoft Office suite. • Excellent oral and written communication skills. • Ability to read, write and speak fluently in English. • Education and experience in the scope of services offered by the agency. • Two years of home care experience including OASIS data collection, preferred. •Maintain confidentiality in accordance with applicable laws and rules. • Provide four week written notice prior to termination of employment.
