As a Physical Therapist, you will provide in-home evaluations, treatments, and education to help patients improve strength, mobility, safety, and independence. You will work closely with nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team to ensure comprehensive, patient-centered care. Minimum of two years of experience in physical therapy; community/home health experience preferred. Strong communication, organizational, and problem-solving skills. Valid driver’s license with reliable transportation.
