Job Summary:

In collaboration with a physician, diagnoses and treats certain acute or chronic health conditions in specific populations.

Job Responsibilities:

  • Obtains patient history and performs assessment via observation, interview, and examination.

  • Orders, performs, and interprets diagnostic studies.

  • May be required to perform preventative health assessments, screenings, immunizations, and care.

  • Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.

  • Provides counseling and education of patients and their families/caregivers concerning preventative health, treatment options, and community resources.

  • Provides care in the context of promoting age and culture-appropriate informed choices, shared decision- making, and the right to self-determination.

  • Provides accurate and timely documentation of assessment and patient care plan.

  • May coordinate the registration functions including collecting and entering demographic, insurance, employer information, and co-payments.

Skills and Attributes:

  • Requires critical thinking skills and decisive judgment.

  • Works under direct supervision.

  • Must be able to work in a stressful environment and take appropriate action.

  • APRN - National certification from American Academy of Nurse Practitioners (AANP) required upon hire OR

  • APRN - National certification from American Association of Critical Care Nurses (AACCN) required upon hire OR

  • APRN - National certification from National Certification for the Obstetric, Gynecologic, and Neonatal Nursing Specialties (NCCCOGN) required upon hire OR

  • APRN - National certification from Pediatric Nursing Certification Board (PNCB) required upon hire OR

  • APRN - National certification from American Nurses Credentialing Center (ANCC) required upon hire OR

  • APRN - National certification as Neonatal Nurse Practitioner from National Certification Corporation (NCC) required upon hire
