Position Summary
Universal Pro Care Inc. is seeking a dedicated and detail-oriented Program Supports Coordinator to assist with the coordination, implementation, and oversight of services for individuals with intellectual and developmental disabilities. Services are not limited to Day Habilitation, Community Development, Personal Supports, Respite, Etc.

The Program Supports Coordinator serves as a key liaison between individuals receiving services, families, direct support professionals, community partners, and agency leadership to ensure high-quality, person-centered care and compliance with applicable regulations.

The ideal candidate is organized, compassionate, and committed to promoting independence, community inclusion, and positive outcomes for the individuals we support.

Essential Duties and Responsibilities
  • Coordinate and monitor the implementation of Person-Centered Plans. 
  •  Liason with the Activities Coordinator to ensure activities and services are delivered in accordance with organizational policies, state regulations, and funding requirements. 
  •  Assist with organizing and scheduling events, staffing coordination, and service delivery oversight. 
  •  Monitor program activities to ensure participant health, safety, and well-being. 
Client Support & Advocacy
  •  Serve as a primary point of contact for individuals receiving services and their families. 
  •  Advocate for individuals' rights, preferences, and goals. 
  •  Assist individuals in accessing community resources, employment opportunities, recreational activities, and healthcare services. 
  •  Support individuals in achieving personal, vocational, and independent living goals. 
Documentation & Compliance
  •  Maintain accurate and timely documentation, including progress notes, incident reports, assessments, and service records. 
  •  Conduct regular audits of client files to ensure compliance with state and agency requirements. 
  •  Assist with quality assurance reviews. 
  •  Ensure confidentiality and compliance with HIPAA and other applicable privacy regulations. 
Staff Support & Training
  •  Provide guidance and support to Direct Support Professionals (DSPs) and other program staff. 
  •  Assist with onboarding, training, and mentoring new employees. 
  •  Promote best practices in person-centered service delivery. 
  •  Participate in team meetings and interdisciplinary planning sessions. 
Qualifications

Education

  •  Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation Services, Healthcare Administration, or a related field preferred. 
  •  An equivalent combination of education and experience is preferred. 
Experience

  •  Minimum of two (2) years of experience working with individuals with intellectual and developmental disabilities, behavioral health needs, or related populations. 
  •  Experience with Maryland DDA services and regulations preferred. 
  •  Previous experience in case management, program coordination, or residential services preferred. 
Knowledge, Skills, and Abilities

  •  Strong understanding of person-centered planning principles. 
  •  Excellent communication, organizational, and problem-solving skills. 
  •  Ability to manage multiple priorities and deadlines. 
  •  Proficiency in Microsoft Office Suite and electronic documentation systems (iCare). 
Physical Requirements

  •  Ability to travel to residential sites, community locations, and meetings as needed. 
  •  Ability to lift up to 25 pounds occasionally. 
  • Ability to drive long distances and large-sized passenger vans
  •  Ability to perform duties in office, residential, and community-based environments. 
Required Credentials

  •  Valid driver's license with a satisfactory driving record. 
  •  Reliable transportation. 
  •  Successful completion of criminal background checks and other required screenings. 
  •  CPR/First Aid Certification (or ability to obtain within designated timeframe).
  • Ability to pass random drug and alcohol screening/ tests.
