Position Summary

The Occupational Therapist (OT) evaluates, develops, and implements individualized therapy programs that help residents achieve their highest level of independence and function. Working collaboratively with residents, families, physicians, nursing staff, and the interdisciplinary care team, the Occupational Therapist provides evidence-based interventions that enhance residents' ability to safely perform activities of daily living while improving overall quality of life.

Key Responsibilities

• Complete comprehensive occupational therapy evaluations to identify residents' strengths, limitations, and functional goals. 
• Develop, implement, and modify individualized treatment plans based on resident needs and physician orders. 
• Provide therapeutic interventions to improve activities of daily living (ADLs), upper extremity function, cognition, balance, mobility, and adaptive skills. 
• Monitor resident progress, reassess outcomes, and adjust treatment plans as appropriate. 
• Educate residents, families, and caregivers on therapeutic techniques, adaptive equipment, home safety, and strategies to maximize independence. 
• Collaborate with physicians, nursing staff, therapy team members, and the interdisciplinary care team to coordinate resident care. 
• Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory and professional standards. 
• Participate in care conferences, quality improvement initiatives, and interdisciplinary team meetings. 
• Ensure therapy equipment is maintained in a safe, clean, and functional condition. 
• Promote resident safety, dignity, independence, and person-centered care throughout the rehabilitation process.

Requirements

Required Qualifications

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program. 
• Current Occupational Therapist license in the State of Kentucky. 
• Current CPR certification (or ability to obtain within required timeframe). 
• Strong knowledge of occupational therapy evaluation techniques, treatment interventions, and rehabilitation principles. 
• Excellent communication, interpersonal, and organizational skills. 
• Ability to work independently while collaborating effectively within an interdisciplinary healthcare team. 
• Commitment to compassionate, resident-centered care. 
• Ability to meet the physical demands of the position, including lifting up to 50 pounds, prolonged standing and walking, bending, pushing, pulling, transferring residents, and assisting with therapeutic activities. 

Preferred Experience

• Previous experience in skilled nursing, long-term care, inpatient rehabilitation, or post-acute care. 
• Experience treating geriatric residents with orthopedic, neurological, cardiac, and medically complex conditions. 
• Knowledge of Medicare Part A, Part B, PDPM, rehabilitation documentation, and electronic medical records (EMR). 
• Experience participating in interdisciplinary care planning and quality improvement initiatives. 
• Strong clinical reasoning, time management, and problem-solving skills. 
• Passion for helping residents achieve their highest level of function and independence.
