JOB SUMMARY
An Adolescent Navigator is an individual who assists in addressing all the ancillary concerns and needs of the adolescent by providing ongoing support, needs identification, intra-agency, external coordination in conjunction with the established treatment team. This employee is responsible for ensuring that supportive services and follow-up are provided prior to discharging a patient/participant from a CBI Behavioral Health Services. The goal of the Adolescent Navigator is to enhance the likelihood of success, identify and reduce barriers for clients receiving Medication Assisted Treatment (MAT) and BH services. The Adolescent Navigator will provide support and ongoing patient engagement while adolescents are participating in CBI Services. The Adolescent Navigator is responsible for participating in Child and Family Teams (CFT), providing administrative support to the Licensed Practical Nurse (LPN) and Nurse Practitioner (NP), coordinating referrals, inclusion of family supports in treatment planning and connection/coordination to community supports.

Experienced Required
ā¢ Highschool diploma or GED and either:
Ā Ā Ā 18 credit hours of post-high school education in a field related to
Ā Ā Ā Ā Ā behavioral health and two years of full-time behavioral health workĀ
Ā Ā Ā Ā Ā experience; or
Ā Ā Ā four years of full-time behavioral health work experience.
ā¢ Has an associateās degree and at least two years of full-time behavioral healthĀ
Ā Ā work experience; orĀ
ā¢ Has a bachelorās degree in a field other than behavioral health and at least oneĀ
Ā year of full-time behavioral health work experience; or
ā¢ Has a masterās degree or bachelorās degree in a field related to behavioralĀ
Ā health.
ā¢ Minimum of six months of recovery from substance use and/or mental healthĀ
Ā disorders required.
ā¢ 1-3 years of substance abuse/mental health recovery-oriented service isĀ
Ā preferred but not required.
ā¢ Experience working with adolescents preferred.

LICENSES, CERTIFICATIONS, CREDENTIALS, REGISTRATIONS
ā¢ Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-115 andĀ
Ā CBI Clinical Policies and Procedures.
ā¢ Current AZ Driverās License (valid and in good standing).
ā¢ Clear 39-month Motor Vehicle Record.
ā¢ Arizona Fingerprint Clearance card, Level 1
